To craft a stone Axe you need to open up your menu and go into the Crafting area. Once in the crafting area you want to use LT and RT to navigate the crafting area to the tools which is a screwdrive and wrench crossed, the stone axe will be under this category, you will need to collect (with your fists) 2 fiber, 2 wood, and 2 small stone (you can find on ground or just hit RT on a rock until you have 2).
Just hit
on the Stone Axe, then 
on the D-pad and it will automatically craft, once done the achievement will be yours.

This achievement will likely be one of the first ones that you unlock in the game. To eran it, you simply have to craft a Stone Ax, which requires 2 small stone, 2 plant fibers and 2 wood. Stones you can find scattered about on the ground which you can pick up by pressing 
while looking at them or you can just go hit a rock for about a minute. Plant fibers you get by punching the grass by clicking 
while looking at tall grass. Wood can be obtained by walking up to a tree and pressing to punch it.
After you obtain all of this just open you’re crafting menu by tapping 
, clicking on the tab at the top left that has the picture of a screwdriver and wrench, selecting Stone Ax, and then pressing 
to start crafting. Once you have crafted the ax, the achievement will unlock.

Crafting a Stone Ax will be the first objective of Noah's tutorial when you first load into your world. Start by getting some wood by punching either a tree or a small twig to get at least two pieces. Then, look around on the ground for small stones that will be littered all over the map. Pick two of these up. You'll also need plant fibers so punch some grass and get at least two of them. After you have two of each material, open up your crafting menu by pressing the 
and navigating to the "hammer" icon in the upper left corner. Under the basic's menu you will see a "Stone Axe". Simply click on it and press 
on the directional pad to craft it. After the axe appears in your inventory you'll be awarded the trophy.
