Starting an online game you will automatically have the bandage and First Aid in your inventory, I don't recommend just walking up to an enemy and letting them beat you to death but try to keep it for when you notice your bleeding out.
When you are bleeding out your health will be dropping at a quick rate so keep an eye on your health after any time you take any damage. There is also a status (little + symbol above your stamina) so if you see it, simply use a bandage by using RB/LB to navigate your quick item area if its in there then hit LT to use it. OR if its not in that area you can open your inventory, Hit

on D-pad to use.Once that is done the achievement will be yours.
Seems as though according to Takeshi117 your achievement will not unlock unless you have your settings on your xbox set to English USA.....

To get this with little effort you can craft a bandage with cloth fragments from collecting cotton. You will then just need to collect some wood to create some wooden spikes.
After that just place down the spikes and then jump on top until you are almost dead and your character is bleeding out and an icon of a heart with an arrow through it.
If you also manage to acquire some broken glass you can eat, yes eat, the broken glass to suffer bleeding damage but will most likely cause death as you take over 150 damage.

In order to get this achievement you have to use a bandage, first aid bandage, or a first aid kit to heal yourself while you are bleeding. When you make a new character, you already start off with a first aid bandage, so all that is left to do is to start bleeding. The easiest way to make yourself bleed is to loot around until you find either a glass pane or crushed glass (if you find a glass pane then you can make it into crushed glass).
Once you have your crushed glass you can select it by pressing 
while your cursor is over it and then you can eat it to make yourself bleed. After you start bleeding, use the bandage and the achievement should unlock after you've stop bleeding.

A "critical bleed-out" will happen if the player gets hit by a zombie too much or stabbed with something sharp such as a cactus or wood spikes. This trophy will come naturally as you play the game.
If you would like to get this as soon as possible then load in a new world with any character. You'll spawn in with at least one "first aid bandage". Then, go and harvest some wood by punching a tree and get at least 4 pieces of wood.
After you have the material, head into the crafting menu by pressing the 
and navigating to the "hammer" icon in the upper left corner. Once here, find the "Tools & Traps" menu marked with a wrench and craft the wood spikes. Place them in your toolbelt & then on the ground in front of you and walk into them a few times to start a bleedout. Once the game warns you that you're bleeding, simply use the first aid bandage before you die. You'll be awarded the trophy as soon as you stop bleeding.
