This takes place in the Middle East level. You'll know when this is, as you're near a crashed chopper and all bandaged up. You will need to find the entrance in the lower section of the map in the second area. Inside will be a lot of zombies. You will need to navigate your way outside to another area which doesn't actually progress the mission, but there is health and enemies around.
Once you're outside, you will unlock the achievement.
