*MISSABLE*
There are three types of collectibles in this game: Hidden Clue Symbols (which are just question marks, akin to the Riddler symbols from Batman), Asylum model fragments (which are objects you can pick up like any normal objects in the game), and Phantom Objects (which are morphing objects). As you can tell from the descriptions, you only need to find three of each. There is one hidden object per scene, so every time you come to a new scene, make it a point to find the collectible there before proceeding.
For the locations of all collectibles, refer to THIS GUIDE over on Steam. It shows every collectible, but once you unlock these three achievements, there's no need to keep it open and find collectibles anymore.

Patch 1.01
It is possible to catch a bug if you skip how you pull the string on the first hypnotic doll.
Then you won’t see more than one phantom object; the game apparently thinks that you haven’t entered hypnosis and don’t see them yet.
Here you just need to start the game again and do not reset the screensaver from the first doll; we pick it up from the floor after the first room with the search for evidence.
For some reason, the clinic’s layouts also don’t appear, as do the “question marks.”
