Upon reaching the hospital, take the elevator up. Inside of the waiting room, you can find a tv in the corner which you will want to turn on. Once it's turned on, have both characters sit on the couch and wait for about 30 seconds. Vincent will have a dream about the moon and the achievement will unlock.

This video guide contains all achievements / trophies in A Way Out in chronological order. Nothing is missable, you can redo anything via chapter select and unless written otherwise, both players earn the achievement / trophy at the same time (They can only be earned with a full copy of the game, not the free friend pass). This game can only be played with 2 players, there is no Singleplayer mode.
#1 The Dip - 0:25
Chapter: Escape - The Yard
Leo must do 20 Dips in a row on the prison yard.
#2 Freedom - 1:29
Chapter: Escape - Prison Escape
Free the Caged Parrot in the Warden's Office.
#3 Take a Breather - 2:49
Chapter: Fugitives - Breather
At the start of the chapter walk to the right. There's a rock both players need to sit on to enjoy the view.
#4 In Sync - 3:49
Chapter: Fugitives - The Farmstead
Go into the house of the old couple. Vincent must play the piano and Leo the Banjo. Each player must hit 20 notes in a row without missing a single one. If one player messes up you have to get another 20 correct notes.
#5 Timeless Treasure - 5:11
Chapter: Fugitives - The Farmstead
Go into the house of the old couple. On the ground floor is a clock, both players need to interact with it at the same time. This opens a secret door upstairs, go into that hidden room to pick up a sword from the chest.
#6 Take it for a Spin - 6:03
Chapter: Fugitives - The Farmstead
Climb the windmill and interact with it a few times to spin it up.
#7 No Cheating - 7:03
Chapter: Preperation - Reunion
Talk to the woman with the baseball bat in the starting area of the chapter. Then go to the trailer to her right side to find her husband. Then talk to her again to get the trophy.
#8 You Started It - 8:19
Chapter: Preperation - Reunion
Both players need to sit on the swing set at the same time.
#9 Home Run - 9:16
Chapter: Preperation - Reunion
At start of chapter, head to the right. There's a guy with a baseball bat. Talk to him and then try to hit a Home Run. Swing the bat as soon as the pitcher starts the throw-animation. You must hit the ball in a straight line over the fence really far away. Only unlocks for the player getting the home run, the 2nd player has to do it again.
#10 Backseat Mechanic - 9:58
Chapter: Preperation - Reunion
Vincent must bring a Wrench to the woman repairing a bike. Then help her start the bike. Leo should stay inside the trailer until the trophy pops and not progress any further (unlocks for both players).
#11 Managed Anger - 11:23
Chapter: Preperation - Violent Questioning
Talk to the guy on the chair and choose to push back the chair. Leo must then pick the purse from the table, drop it on the floor, and lean over the table. Vincent must then use an object to torture the guy which triggers a dream sequence.
#12 Live the Dream - 12:58
Chapter: Preperation - A Nie Life
After riding the elevator both players must sit on the couch in front of the TV. Watch TV until Leo falls asleep.

Chapter: Vengeance - Lift Off
Both players need to play the arcade at start of chapter.

Leo has to interact with the Helicopter at start of chapter.

Subchapter: A New Life
After reaching the hospital section of the game, you'll get out of an elevator where you have a special encounter with a guard and find yourself in a waiting room. Here, you will need Leo to walk over and turn on the TV and just sit down. The other player can walk about freely until a cutscene plays and you'll be in a dream on the moon where the achievement/trophy will unlock.

Chapter Select: Preparation - A New Life
Trophy unlocks for both players? Yes
Near the start of this chapter, you'll enter an elevator and a guard will run in along with you. You can make a choice of Vincent's way to talk your way out of it or Leo's way to knock him out. Either choice will work.
Before doing anything else, ignore the current objective of finding Carol. Do the trophy requirement by going into the waiting room. In the waiting room is a television that you can turn on. It will show an image of the first man landing on the moon. Both players need to then sit on the green couch in front of the television and after waiting for a few minutes. A brief cutscene will trigger for the player who's sitting on the left side of the couch. They will fall asleep and will be on the moon. Once you're in full control of your character on the moon, the trophy will unlock. Either player can have this dream, but it will only trigger for the character on the left of the couch.
If Vincent speaks with the receptionist to gain access to Carol’s room, then you've gone too far and will need to use Chapter Select.

Once in the hospital, take the elevator up to the maternity ward. Once in the maternity ward waiting room you will see a TV, have the player controlling Leo turn on the TV then have both players sit on the couch across from it. After a couple of minutes Leo will drift off into a daydream about him on the moon, unlocking the trophy for both players.

This unlocks during the chapter ‘Preparation: A New Life’.
After getting out of the elevator at the hospital, turn left and then turn left again and you will enter a waiting room with a nurse’s station. Before speaking with the nurse, turn the TV on in the corner and have both characters sit on the couch. Whoever is on the left will doze off after about 30 seconds and will have a dream about being on the moon (mirroring what is being shown on the TV). When you have control of your character, run towards the moon lander and the achievement will unlock along the way.
Note: If you speak to the nurse before you do this, you’ve missed your opportunity and need to restart the chapter.

This trophy is made as follows:
Leo turns on the TV, sits down on the sofa and no more manipulations for him.
After this, Vincent goes to the nurse at the post and asks about his wife. The nurse opens the door for him, Vincent stops and calls Leo (Leo doesn't have to do anything). Vincent approaches Leo, a cutscene begins, after which the trophy is yours.
