After exploring Flathill (portal) There is an event where the facility becomes foggy which causes the "Symphonist" to appear,
they're pretty hard to kill by yourself, so teaming is recommended. Once killed they'll either drop (don't know if there is drop%) partial organs or a Symphonist head. Using the Symphonist head, near the the entrance of the Wildlife Pens will be a person named Marion who you can trade the Symphonist head for a Porcelain Key. Head up to the 2nd floor and start walking to the laser which is coming out of the pipe. Here, you'll find a door which you'll need a lvl 2 hacking device. After opening there will be a hallway with boxes blocking the way, breaking the boxes and infront of you will be the Cacophonous Crate, opening it will get you this achievement.
