Pretty simple, while swimming underwater, start towards the surface, and just before reaching the surface, press the boost (left click for mouse) to leap out of the water.

This trophy can be obtained at the very beginning of the game. All you need to do is swim down then hold the boost button and shoot up towards the top to breach the water. If you happen to miss your chance at getting this in the beginning of the game, there are quite a few areas that will allow you to go up to the surface and try at this trophy again.
