Turns out you have to 5 star all the songs on the *same* instrument and on the *same* difficulty level. e.g. all songs on Hard Guitar/ all songs on Easy Vocals/ etc.
Evidence for this comes from my own experience of playing through career on Hard Guitar and missing the fifth star on 'Back in Black'. I then 5 starred it on Hard Drums, but no achievement. I returned to Hard Guitar, got my 5 stars and the achievement popped.
May just be a glitch - but I'd recommend doing your first playthrough on an instrument and difficulty you are confident with to avoid getting stuck with having to play all 18 songs again!

Seems to make most sense to sit down and get the "play in one session" achievement with this one, won't be as tempting to change difficulty, switch instruments, etc.

just yesterday i did this in quickplay mode and it did not unlock, now gonna try in tour

Any difficulty means any difficulty, so if you prefer playing on a lower difficulty you can still can obtain this.
This can be done in Quickplay or Career mode.
