In The Confession, there's some meat in the back of the van. Pick it up and feed all of it to the pigs. Make sure the conversation doesn't end before you get all of it. You can choose to not answer to stop it from advancing.
When the pigs eat all the meat, this will pop.

I found this easier to do this one by one and not throw the next piece in until the pigs had eaten the previous one - sometimes it can glitch and the pigs won't eat one if there's too much according to reports on Steam. Unsure if this follows onto Xbox, but decided to play it safe - also helped to know exactly where I was with this achievement.

*START HERE*
When the game opens, you are sitting on your porch and see a white van pull up. Press
to open your journal then again to close it, then 
to move.
Scene - “The Confession”
Standing there talking to the man, press
to skip through the dialogue. When presented with a choice, instead of selecting a reply, walk over to the back of the van and you will see 10-15 packages of meat. To unlock this, we need to throw all the packages into the pig pen. Pick one up with 
and walk to the edge of the pen. Aim above the group of pigs and press to throw the package. Repeat until all of the packages have been thrown into the pen. Once all the meat is eaten, the achievement will unlock.
The next achievement you will unlock is Piano Man (30G).
