Very straight forward and is reached by progressing the main story line. There are a total of 5 outposts to activate with Finn. These are the posts which display all the treasure chests on the map. Here is a list of locations:

Candy Kingdom - 1
Evil Forest - 2
Fire Kingdom - 1
Firebreak Island - 1

NOTE: This did not pop for me right away. I had to log out to the title screen and back into the game for it to register. This is a glitchy achievement. If it happens the achievement does not show up once the last post is activated, then quit to title and come back in.

Glitched. I even ran through all the lookouts a second time to make sure. It didn’t pop for me. Wish I would have read this first.

This is not story related as mentioned above. You can finish the story without activating a single Pirate Lookout, although some of them are very hard to miss.

The Pirate Lookouts are located in the following locations:
Candy Kingdom - 1
Evil Forest - 2
Fire Kingdom - 1
Firebreak Island - 1

There are a total of five pirate lookouts. The trophy is bugged or glitched in that you might not need to use all of them in order for it to pop.

Candy Kingdom
-- This is revealed shortly after interrogating Admiral Candy Corn on the right side of the walkway.

Evil Forest
-- After interrogating Peppermint Butler, there will be a route highlighted by Marceline. Further in is a Trigger Action for Jake that leads up to this lookout.
-- Returning to search for Fern, you'll rescue BMO. Use him to open the gate to the left of the cage he was just in (on the walkway) to find this lookout and open the door back to the boat.

Fire Kingdom
-- After passing Cinnamon Bun, turn left and use Jake to get to the higher ground. Immediately turn to your left to find the lookout.

Firebreak Island
-- As you're collecting the cores for Torcho, you'll pass right by this lookout.

Pirate lookouts are telescopes that are usually located on high points of whatever island you're on. The first one you come across is right next to where you will interrogate Admiral Candy Corn in the Candy Kingdom. Simply walk up to it as Finn and interact with it.

There is one in the Candy Kingdom, two in the Evil Forest, one in the Fire Kingdom, and one on Firebreak Island (it only seems to spawn when you need to go to the island for story reasons). This trophy may be glitched in a good way, as I unlocked it before I even got to the Fire Kingdom.

Pirate lookouts will be shown on your map – the icons are brown and kind of look like a germ (I guess it's supposed to be an island?). It is in your interest to use these regardless of the fact that there's a trophy for doing so – they will put the location of every treasure chest on your map (Dosh Ahoy), and (I think) side quests.

Pirate lookouts are for Finn's special land ability. You want to look out for a pirate flag and telescope. Stand in the glowing circle and use to activate these lookouts for the island. This will show you the treasures on your map you don't have. Collect them all; opening a few doesn't hurt for money or more abilities.
