You must get a full game going for this (4 people including yourself).
All 4 ships must attach together, it will pop as soon as you 4 are connected so there is no need to stay together if its not wanted.
To attach to another ship simply get close to it and press

You can usually announce you need the achievement and everyone will come together or you can get 3 friends to help you out.
Also if you happen to have 3 extra controllers you can do this yourself locally.

Also add in that one can do so with four controllers, with ease.

This achievement is pretty basic, but must be done with four players. Just do Co-op online, and go next to another player and press 
to attach to him/her, and have everyone else do the same. Once all 4 people are attached into 1, you’ll get the Achievement.

Four players are required for this. This video also includes the hero achievement where you must complete the game on normal difficulty and the diplomat achievement where you must complete a level without firing.
