Just a note, since the achievement descriptions have been translated poorly from their original language.
It doesn't count repairing as getting the floating health-up balloons.
To 'repair' your ship, you must die, and use the crystals you've collected from beating a boss (I believe the first time you revive, it requires five crystals.).
Once you've revived, the achievement will unlock.

Same, I've got crystals ready but it's not giving any options to revive...
Edit: Just popped this, you have to die the end of level boss to get the option to revive!
