Chapter 6
After the scene in which Poirot interrogates multiple passengers in the dining car (and dodges a punch from Arbuthnot for another achievement), you'll be back to freely walking around the train. Go to the passenger compartments (you can stop on the way for a chat with Hildegarde Schmidt and a puzzle involving a Russian matryoshka doll). Knock on the door of room 205 and Count Andrenyi will answer. Your first conversation with him will lead to a character analysis, and it will end with him closing the door.
Knock on the door again right after this, and you will attempt to convince him to let you speak to his wife. You'll have to make several correct dialogue choices to succeed and get the achievement. Make the following dialogue choices:

** Ask for Mr Andrenyi's help

** You love your wife
You protect your wife

Offer his help
** Offer Dr Constantine's help

** You should go get him

This will send the Count off to see the doctor, letting you enter the room to talk to his wife and, more importantly, unlocking the achievement.
