This is unlocked when you get stuck in the elevator in the library, because you have to much stuff in your inventory.
Read elsewhere that you need to fill the first page of your inventory (15 items) for this to happen, but neither that nor 16 items wasn't enough for me.
I managed to unlock this on my first visit to the library during the playthrough where I spent money, and I had the following 18 items in my inventory:
- Box of pens
- Copy paper
- Library access card
- The 8 books from the shelves between the supply room and the photocopier in
the library (these count as 8 separate items)
- Discount flyer for the copy shop
- Lottery ticket
- ID photos
- Money
- Knives
- Hammer
- Bottle with pig's blood

i got this with 16 items in my 2nd playthrough:

You can get this one any time, you just have to get enough things in your inventory that Agatha gets stuck in the library lift.

Hammer (Nika's house)
Bottle with pig blood (probably already in your inventory)
When you have all of this items, get into the lift and Agatha should get stuck.

This achievement requires you to have 16 items in your inventory (even though the max slots are 15) and then use the elevator in the library. It'll get stuck halfway up, trigger some dialogue, and then this achievement will unlock.
The easiest way to build up that many items is to do it pretty early on in a playthrough. If you're following the walkthrough, here are the items you can have when you get to the library:
