Can be unlocked on any Difficulty but you must go back to Main Menu after completing the Mission for the Achievement to unlock.

For this to unlock I had to quit to chapter menu after completing the said mission. I played on Standard

Same thing here. I had to go back to the main menu and it unlocked there.

Story mode does unlock this achievement, but you have to back to main menu in order for it to pop.
