This didn't unlock for me via story related activity. It unlocked for me by going to the map and finding a Legion Lair marked on the map. Going to the location than scan for the item to be hacked. This reveals the lair entry go down and complete and achievement should pop.

It unlocked the same way for me, I am unsure if it is meant to pop via story progress or not, but given some of the others such as destroying a golem, ice barrage encampment etc only popped when they were done in freeroam, I would assume it is a freeroam thing.

unlocked for me also doing a lair in free roam, so I don't think its story related

They look like this on the map:
You will complete a lot of Legion lairs during the story but they do not count for this trophy. Open your mission menu when you're in Seoul and search for a Legion lair. Go in this location, scan and hack the entrance and complete the lair. If it doesn't appear on the map, you need to take over an outpost to make them appear.
