There are a lot of things that you can/need to hack in Seoul and inside lairs. When hacking, you need to press 
when the cursor is in the orange or yellow zone. To get a perfect hack you need to press when it's in the middle of the yellow zone, but you will need to do that for all the nodes for it to count, not just one.
If a hack has 3 nodes and you get only 2 perfect hacks, it won't count. If you perfectly hack the 3 nodes, it will count as 1 perfect hack. Do this 15 times to get the trophy.
