Edit: it's possible that lairs are randomly generated which if true renders the guide not as useful.
Best advice for finding the secret room is to kill all the enemies in each room and then check the perimeter for a room off the beaten path - before heading to the objective.
Once in the secret room, search around for the secret in the secret

I did this on the "Special Investigation: Yeti" mission available near the start of the game. This is the mission you must complete before you get the chance to play a new agent - yeti in this case.
During the mission you have to enter a legion lair and once you clear each room of enemies, go for a little stroll around the perimeter before moving to the next area.
The "secret room" was as simple as a door that wasn't the main exit - which wouldn't open till you murderised all enemies. Enter, loot chests, climb up and explore a little more before finding a larger area.
The video below shows the room in question - after all the bad guys were dead. The secret room is when I go off the beaten path to the right to find a side room instead of going to the objective marker. The side room will look different for you because once you enter it you have to climb up before the walls fall down and reveal the room to be bigger.
Ps once inside have another little exploring to find another hidden nook for the secret in a secret cheevo. In the video the second achievement is found in the area at the end when I start shooting my guns.

Yeah not heading straight for the objective was my general advice. Shouldn't be too tough.

They spawn on all lair missions. It's a hole in a room off the beaten track.
When you clear a room of enemies you look for a room away from the waypoint and inside a small room you find a hole in the ceiling. I used Braddock to climb up and got the achievement when she got up there.
