The Wyrm Queen is a boss of sorts located on Eastern Island, which is literally east of your starting island. You will first need to reach the island, get a quest from the Alchemist to follow wyrms underground, then enter into the 'Maw' entrance you find.
It is STRONGLY recommended you have a lantern with fuel, as there is no light source in the Queen's area and torches don't give as much visibility.
The Queen itself is a hopped-up, massive Wyrm in a small area. She only performs 3 steps: Move forward, turn around, and occasionally open her maw to disgorge a new Wyrm. You can get damaged if you stay on her mouth when it closes, although whether it's from her or from the created Wyrm is unknown. Otherwise, she is mostly harmless and her disgorged Wyrms are a source of harvestable dragonblood. On the other hand, she's well-armored, and the only effective way to actually damage her is to wait until she opens her mouth to spit out a Wyrm. Attack her mouth when she does to harm her. She only has 12 points of health, and as she has no effective attacks, she should die easily to you.
Once slain, she will be gone for good from the chamber, and cannot be used as a Wyrm-spawner for easy dragonblood. However, you will obtain this achievement, and a new, smaller 'maw' portal will appear in her chamber. This leads to a random tunnel maze with unbreakable tiles. You can follow them to find maws leading to other chambers, some of which may have respawning Queens that you can use to farm dragonblood again.
