In order to unlock this ending and achievement, you have to follow the "Spin the dial" route on Day 3: Sunday: PSYNCIN' IN THE VaiN. Eventually, the route will become locked until you learn more of the story. For me, I had done the Mizuki, Ota, and Iris endings, and then started my way towards the True Ending until that route locks - that's when this route unlocked. From there just finish playing through until the ending.

This trophy is unlocked by making it to the Annihilation ending. You must follow the following paths in each branching Somnium you reach in order to get this ending:

- PSYNCIN' IN THE PaiN (PaiN: Mother, Blood, and Secrets
)- Route 1 - PSYNCIN' IN THE VaiN (VaiN: Disassociated Persona )- Route 2
While I haven't experienced this personally as I had done these before following this route, it also seems that you require some information from the Iris Route: END 
and some of the Resolution Route: END
