This achievement is story related and can't be missed. It's the True Ending Route, which you can only get to after you've completed every other route in the game. The achievement will unlock once you've played through the Epilogue and after the final credits.

This is the true ending trophy. You basically have to do every other route in the game to unlock the next part of the true ending route.

This trophy is unlocked by making it to the true ending of the game. You must follow the following paths in each branching Somnium you reach, plus obtain all other main endings in order to get this ending:

- PSYNCIN' IN THE PaiN (PaiN: Mother, Blood, and Secrets
)- Route 2 - PSYNCIN' IN THE MaiN (MaiN1: Antinomic Conflict ) - Route 2
See each linked trophy for walkthroughs on following the correct route for each Somnium. As mentioned, even if you take the correct path you will be be locked out of completing this path until you've completed the Ota, Mizuki, Annihilation and Iris endings so see the guides for those endings if needed.
