On the main menu, move your cursor with 
and hover over your profile picture in the top right. It’s the icon to the left of the settings cog icon, and for me was an “N” in a circle since my gamertag starts with an “N”.
Then, hover over that enlarged icon on the next screen, and from there, select any Avatar, and scroll to the Backgrounds & Titles tabs using the triggers. Change them to anything and you’ll pop the achievement.

You don't need to change the Title (none are unlocked at the very start), just view that screen after changing the profile pic and banner.
