On any level, crash your plane and hit retry. Repeat four more times. Achievement pops on reload.

Seems like you need to start any level from the selection screen 5 times. I believe this needs to be the same session. I swear I had to do it 6 times. I did this at the end with level 5 and blew up all of the buildings with the heavy bomb.
