I don't know if this is random, but I found this charm after I completed the nursery rhyme located near the Valhalla Nursing Home, on path that leads back to the break room, in chapter V Old Gods (Saga).
Simply put it on one of the bracelet's slots and just die.

There are multiple Coffee Mug Charms in the game, awarded by solving Nursery Rhyme mysteries. You can do this only as Saga. This is unmissable, as you can trigger this at any time before point of no return.
TL;DR Find Coffee Mug Charm by solving Nursery Rhymes. Equip the Charm. Run out of health in combat - done!
First opportunity is once you beat Nightingale in the Overlap and find our other hero on the beach. Flooding has receded in Cauldron Lake, so you will receive the optional mission objective to search the area revealed by the flood. Do this, and make your to the Witchfinders Station, in the northern part of Cauldron Lake. In front of the station, you will see the rhyme. In the building, you will find the Hero and Wolf dolls. Place the Hero on the Boat, and the Wolf on the Tree, follow black tracks, and find the Coffee Mug Charm upstairs, inside the play pen.
In your inventory, slot the Charm in your bracelet. Now all you need to do is to run out of health when fighting against Taken, you will find many opportunities during the game, but closest almost guaranteed spot would be the General Store. Instead of dying, you will immediately gain full health and keep fighting - Coffee Mug Charm is consumed, and trophy is unlocked (you can also make a manual save before "dying" if you want to save the Charm, but you are able to find more later).
