After dropping down into a small area, you'll have a Train Engine Car and its fuel car come alive and attack you. These cars have additional protection that can only be hurt by focusing your flashlight or using Flares/Flare Gun shots.
You MUST destroy both train pieces to be rewarded this. I simply got the key, unlocked the door, and ran away without fighting it and did not unlock the achievement.
