How to unlock the Iron Will achievement in Alan Wake - Definitive Guide
This is obviously quite full of spoilers. Please be careful when reading!
Being ‘severely hurt’ means having less than a quarter of your health left. This is when the game becomes black and white, and your heart beat becomes loud and erratic. As the description reads, you must avoid this state of health throughout the final encounter.
The final encounter comes in several stages. You will initially fight Barry on the bridge heading towards Bird Leg Cabin, though you only need to shine your light on him at this point. From there, Barry will send several waves of familiar faces after you, in the form of speedy Taken (First Hartman, then the Anderson brothers together). Each enemy is accompanied by pesky birds. Since these are speedy Taken, who often keep their distance until you’ve removed the darkness from them, it is possible to focus on the birds initially, as they can whittle away your health fairly easily if left alone. If you have spare flares, they can help you easily deal with the birds. There are ammo crates for the revolver, shotgun and batteries that you can use to stock up when necessary.
A tip that worked for me when needing ammo was to remove the darkness from an enemy, shoot them a few times, and stun them with your flashlight, while moving into position near an ammo crate. This gives you a brief period in which you can restock. After fighting Hartman and the brothers, you will face Barry three more times, with the last encounter being a speedy Taken form. Each ‘wave’, so to speak, plays out the same, so find a way of dealing with the encounter that suits you and stick with it. Just remember; deal with the birds first, then the Taken!

Die and reloading works, Barry himself was easy but the 1st section took a couple of do overs.

Apparently if you die it doesn't count as being seriously hurt. I died during the part with the brothers and I still got the achievement. I didn't reload a save or checkpoint or anything. After the death I never was seriously hurt and I got the achievement after defeating Barry. On another note, as NorrisMM points out, the flares are great. If you need more ammo you can throw a flare next to the ammo crates and restock without worrying about enemies.

what also helps is if you do get seriously hurt just reload last save point. it depends on what part of the fight you're on but it should only take you back to the start of the part of the fight that your on at that point. just watch out for the birds they'll get you every time!

Just wanted to note that this helped a lot. Sure, you can't do this if you going for the No Punctuation achievement (and you'll probably get this achievement with that one any way if you are). But as someone who wants to do No Punctuation -one day- without any other achievements hanging over my head during the DLC I just went ahead and got this one right away.
This might be one of those "mileage may vary" things but when reloaded during the fight it took me back to when the 2 brothers started attacking me, and no matter when I reloaded after, even during the last phase vs. the main opponent, I'd go right back to that spot in the fight -which was very helpful once I got a feel for the fight.

This is how it worked for me on normal mode.
When the fight begins, pick up ammo and easy go back to the point, where the BRIDGE was.
You are now with your backside to a stone or something like that and cant get attacked from behind and the sides. Now the only thing is to watch your frontside, using your flashlight and reload it if necessary.

This is part of The Writer DLC.
"Seriously Hurt" means without dropping down to the last bar of health. If you happen to take too much damage, reload your checkpoint and try again. Use Flares when the birds are bombarding you, and just try to take out the enemies as quickly as possible.

You must finish the last scene without going into low health (being in the last quarter of life or lower). You must fight darkness infused characters from the story. Dr. Hartman, the Anderson brothers (both at once), and Barry. They are the in the form of the "fast" taken, that go blurry while moving quickly to different areas around you. Crows will also attack you. Use flares if crows are attacking and do your best to dispose of the enemies quickly. Barry is the last of the encounter, and you will have to essentially defeat him 3 times. If you do fall into low health, I advise to reload your checkpoint and try again.
