Sadly. I don't think this works anymore.

I did this on my second play through with max weapons. Teapot cannon is your best friend. It will kill most enemies with one or two shots. I mainly used on eyepots, madcaps with shields, and the bolterfly hives. Vorpal blade take care of the rest with ease.
Main thing is dodge their hits, their hit points isn't an issue with max weapons.
Hope this helped enjoy.

So I just started this game yesterday and went to nightmare right when I started and right now I am on chapter 2 and doesn't seem that hard so far. So should I just continue or play through on easy first and get better weapons?

Once you start the game, you will be ask which difficulty do you want to play the game on, scroll to the bottom and pick Nightmare. After you have completed the first chapter on nightmare the trophy will pop up. The first chapter is more of a tutorial than anything else. You will gain a few weapons and learn the moves fairly early in this chapter. The best way to conquer this chapter with ease it to use one of the DLC dresses from the PSN store or use the dodge button often to avoid getting hit by any enemies .
Please see Madness Prevails on how to earn this trophy without playing through the whole chapter.

Under no circumstances should you start obtaining this achievement without completing the entire game (if you are playing at weaker levels). I killed two and a half hours on this first chapter, but ended up getting nothing. The gamepad almost flew out the window...along with the game.
