“No one can hear you scream” isn’t a difficulty per se and as such it could be done in conjunction with any difficulty options.
This mode limits your saves.
The only auto saves you get are:
* on the ship
* when resting in safe rooms
The latter will be the majority of your saves but these cost “tools” to weld the door and initiate the save.
A good number of tools can be found on each map and any spares you have at levels end go into your reserves and if you run out of these on home base you can spend resources to buy more.
It’s really not that bad as you should be resting in safe rooms to keep your stress levels down often.
Had I known how to play the game from the start, I would’ve done this mode on my first playthrough as it’s fairly straight forward.
Tips:
* Use stealth (in all play throughs).
* Scavenge (but if there’s too many enemies, it’s fair to let some items go).
* Drop portable motion trackers regularly and far apart so you can activate them and make the xenos run all over the map when they approach your position.

Remember, during NG+ campaign selection, it appears that custom can be selected and the timer can be turned off. So you can skip as many days as you like even after Cerberus Protocol and achievements unlock.
Someone else please confirm.

The countdown difficulty can be customized to no threat, thus no time constraint of days passed on the Otago, IF NOT PLAYING ON A HARDER DIFFICULTY.
My achievement unlocked just fine.
I don't recommend attempting this on NIGHTMARE because that difficulty forces you to have only 20 days remaining after the cerberus protocol attempted deactivation.
You will need to be able to save frequently without having to return your team back to the Otago, because as you know "DON'T LET THE HEAR YOU SCREAM" greatly limits when the game saves.

Did on easy difficulty. Some mssions are little hard, but doable. Resting helps a lot

A nice bonus: saving through resting in brewed rooms is available even with this option. That's why wrenches are your best friend on this walkthrough.
