You'll get challenge cards as you progress through the game and you complete your daily missions.
Next time you are on the lobby screen, equip one of those cards, if you met the requirements by the end, achievement unlocks.
It can be headshot related or some other challenge, just pick what you think you will not fail, in my case it was a challenge that dissabled the health packs, ran the first mission with AI, every one of us had 4 lifes.

These challenge cards do not necessarily have to up the difficulty. There is some that are strictly beneficial and will not give you rewards. Try to NOT use these until you are going for higher difficulty playthroughs as you will need them.
