this isn't an actual solution, but to get each of the difficulty achievements you have to physically complete each stage on the respective difficulty... completing 11 on standard and the 12th on intense or higher won't earn you the achievement. I hope this helps for those who are wondering why the achievements haven't popped

Also, standard is kind of the only difficulty you can run with AI without too much trouble.
