You'll come across your first melee weapon once you get transported to the French Quarter late in Chapter 1, as either Edward or Emily. Head towards the lit-up store that is your objective point, then turn down the road to the right and then swing left into an alleyway behind the store. At the end you'll find a pipe to pick up and a monster will spawn behind you. Use RB to bash it until it's dead and your achievement will pop.

When you visit The Hateful Mound in Chapter 2, your search for the bridge lever will bring you to an area where you'll drop back down to where you came from. Before you drop down, be sure to grab the pipe leaning against the blue oil drum. You'll then immediately encounter a monster – thrash the living crap out of it with the pipe, hammering 
, to unlock this.
