In the KVK Checkpoint, you'll come across a DJ in a booth. Choose the following options:
get in -> yeah -> either do her or just talk
At this point, you'll receive the whistle, and then in the following two prompts, pick whistle each time and the achievement will unlock.

EDIT: I can confirm that also happened to me when I didn't take the KVK Card from the Private Booth. I guess that's a secret requirement, as in the following scenes you use said card, so I'm pretty sure that's why it didn't work for you.

This isn't working.
When outside the booth all choices including do nothing result in goons taking you out to the Chem Plant scene.
