How to unlock the Iconoclast achievement in Amnesia: Rebirth - Definitive Guide
This is for one of the three endings. Fortunately, you do not have to do 3 separate playthroughs to get each of these.
Make sure you create a save at the very end of the game in the crib room. That way you can reload and perform the remaining actions required for each ending without starting over. If you didn't make a manual save, there will most likely be a "Throne Room" auto save when you try to load game which you can use.
For this one, you want to take the baby and leave the Goddess, but also kill the Goddess in the process. To do this, grab the leyden jar in the crib room and head back into the main atrium where the Goddess resides. You will need to evade her gaze by hiding behind that branches and enter the 3 additional rooms connected to the main room and inject the syringe into the system 3 times until the Goddess is killed by the Shadow. Once the credits roll, the achievement will unlock.

After the credits, just press "continue" and you will get back to the crib room. Now you'll be able to get another ending.
NOTE: You NEED TO HAVE an injector AND red liquid leftover from The Lower Factory!

This will be 1 of 3 endings you can get in the game. You don't have to do 3 separate playthroughs for them. The game will autosave right at the final scene in The Tower, allowing you to get them all, one right after the other.
This ending will take you the longest to do and will require you to contaminate the Vitae pumps. There are 4 total, but you already done one previously (where you injected the red gunk into the pump). You should have 3 more red vials in your inventory. You'll also need 1 Vitae chamber to access 2 of the remaining pumps. There is one available outside of one of the pump rooms, but there is also one just to the right of door you exit the nursery through, so it's recommended you pick up that one to save yourself some time. When you're done feeding Amari, don't put her in the crib and instead, grab the Vitae chamber and leave the nursery. When you get outside, the Goddess won't be happy and will try to change you into a Harvester before you can leave/sabotage anything.
Around the room are 3 pump rooms; 2 are locked and will shine red lights and 1 is already open and will shine blue. Inside each of these rooms is a Vitae pump that you need to inject the red gunk into. Unlock the 2 locked rooms with the Vitae chamber you picked up (don't forget to grab it when you leave each room). Be careful as you make your way around and try to avoid the Goddess' gaze. If Amari starts to cry, calm her down by holding . If she cries too loud, the Goddess will focus on your location, making it hard to lose her again.
Once you've contaminated the remaining 3 pumps, a final cutscene will play out. When it's done, the trophy will unlock.
