Fuck sake. Not unlocked for me, either. Tried finishing the game, deleting my console save and resyncing, re booting my Xbox...nothing.

This Guide is to help all people who needs to get other Achievements on the list. Which is Collectibles, Missables Achievements that you can gather on your playthrough and yes the Achievement is still glitched today. So for the few who says that, I do know that the Developers did say something in the steam forms. So it might get patched in the future or not. As they did say that it's not showing up glitched in their systems.

I somehow managed to unlock this achievement despite knowing that this achievement is glitched for most people but I'll try and explain how I got it. First off, I got all achievements in one save file. The order I basically unlocked all achievements were...
1. Did a partial no save run to get all the dog tag codes (Save all of the codes on a piece of paper or on your phone)
2. Reloaded to previous save, which started me at the bunker, and I started the speedrun,no save, no inventory box run (unlocked some misc. achievements in the progress) Don't collect any files in this run
, its important you don't because the Librarian achievement is also glitched.
3. Once finished, load up the completed run and it should load you at the last autosave, after you blown up the exit. This gives you as much time to get all misc, achievements.
4. Finally, I got all the collectibles, I saved at the main generator room will a full generator, and I just searched through the bunker for collectibles. Note, save the files in the main generator room for last!!! Collecting these files last somehow unlocked the librarian achievement for me, along with the all achievements trophy.
Hopefully this helps some people with unlocking this achievement but if this method doesn't work, hopefully Frictional will release the next patch soon.

There are glitchy trophies on Ps5.
Trophy - Librarian / Librarian - for collecting all notes 52 (+16 photos, which do not seem to count) may not drop, launching saves did not help. Had to play the game again. This problem occurs (exclusively, but this is not certain) on nextgen.
