Alright so this achievement requires you to first get two others.
The Secret Achievement achievement in Amnesia: The Bunker worth 52 pointsContinue playing to unlock this secret achievement
The bunny is initially found during the tunnels, once you reach the Roman section and obtain the detonator handle. In the same room that you obtain the handle, there is a small hole in corner of the wall that you can clear. If you crawl through the tunnel you'll end up in an outside area with a lake. The rabbit is beside the lake.

Then you will need to throw the toy rabbit at the feet of the beast and it will examine it for a short time and take it. There were some instances for me where it did not take the rabbit and it will remain on the ground if that happens you can simply try again.
Once the beast has taken the rabbit (you can tell if the rabbit is no longer where you threw it), you will have to head to the Chapel which is accessed via Maintenance, through the Workshop (or via the stair shortcut if unlocked) and next to the Fuel Storage.
The bunny will now be on the altar in the Chapel. Pick it up for this achievement. Please note that if you gave the bunny to the beast anywhere in Maintenance, you have to go back to Admin and then back into Maintenance again for the game to load the bunny into the Chapel.

I've picked up the rabbit from the alter 4x now, (reloading save, throwing it at the beast, acquiring it from the alter, and throwing it again) this just won't pop..

In order to Achieve "Toy Fanatic" you must First Obtain the Rabbit via "Toy Collector" and then apply the Rabbit via "Sentimental".
Doing so will gain you the opportunity to Re-Obtain the Rabbit.

If you've gotten Toy Collector and Sentimentalist, then Toy Fanatic is the final piece of the puzzle.
After the monster has picked up the rabbit and run away, head to the chapel and you can pick it back up on the table. I had a bit of difficulty getting it to show up at first, so if you do run into trouble, leave maintenance completely, wait for the monster to start making noises again, then run back into maintenance and head to the chapel and it should spawn in!
