Level 2 - black camp. Make sure you have one grenade just before starting the mission with the parasites. Once in it, wait for them to be enough and throw your grenade.

It's fairly easy to find groups of 5 or more enemies at once in Street Brawl, but it's not always easy to find the specific items. Black Side mission 3-1x regularly gives you grenades, traps, and large enemy groups, so find a group of 5+ and toss a grenade into the middle for the trophy. The explosion range is huge, so you don't need to be too accurate.

Incendiary grenades are a slightly more rare item than most. However, in Black Side 2-1, the mission with the Gargoyles, there will always be one in the rightmost item box. Simply pick up the incendiary grenades and save it for after the mission. Then in Street Brawl simply throw it at a group of 5+ thugs to unlock this achievement. You select Stage 3 on the black side of Stage Select and there you start the first Free Mission, which takes place on the roof of the building. There you need to destroy 50 enemies within the allotted time. Throughout the mission, two incendiary grenades will drop from the drones, one of which will fall approximately after the first batch of destroyed robots. As soon as you pick them up, wait until the next batch of drones gathers close to each other and throw a grenade into their middle.
If memory serves, just after receiving the first grenade a batch of drones appears literally at one point and all close to each other, so don’t miss the moment.
