This will probably be the last achievement you get. In between runs there is an "Upgrade" menu where you can use the in-game currency (gold) to buy perma-upgrades. You need to buy all three levels of "Cosmos" in order to get the achievement. Gold is earned during your run and you'll need to play probably about 20 minutes or so in order to buy all three levels.
Step 1: Unlock Cosmos by surviving for 5 minutes during a run.
Step 2: Buy all three levels of "Cosmos" for 150 + 225 + 338 = 713 gold

To unlock the ship SS-Cosmos, the ingame challenge is a little unclear, making it sound like you need to fully upgrade a different ship or something. What you actually need to do is fully upgrade the Cosmos weapon in the 'Upgrades' menu. This will cost somewhere around 730 coins to fully upgrade it. You will most likely need to do two runs to get that much money (although it's fully possible in one run if you survive the 15 minutes). Money can be grinded in as short of runs as you need to eventually amass 730 coins or so to fully upgrade the Cosmos weapon, at which point you'll unlock a new ship and this achievement.
