Normal is the second difficulty and is available to you right away -- it has 5 levels, slightly more aggressive enemy patterns, and bosses will have an additional "enraged" phase. Normal mode also introduces some story elements for the character you are using.
Once you have completed the fifth level/"campaign" this achievement will unlock when the game tallies your score.
I recommend not using your points for post-map power-ups or to continue after dying -- the exception being the 1HP if needed.

Beaten this a few times but it has it popped. FML

I have had a ton of issues with the game saving my progress. I will play, unlock a bunch of perm buffs, come back later, and they aren't unlocked anymore. It saved at some point but the new ones don't stay. Curious if anyone else has run across this issues and if there is a solution....
