I got this achievement on the Easy difficulty.
There are 6 potion upgrades in total and are made using mega plums that are randomly placed around the world. Since the game is a randomized there's no specific map to find all the mega plums, you'll just have to explore/open up the map to find them. In order for you to pick them up, you need an open inventory slot. Once you are ready take the plum(s) to the Potion Chef in Fairreach. You can either wait till you have all the plums needed for an upgrade, or take them to the Chef one by one. Additionally, if you have a plum in a chest, you'll have to take it out of the chest to use it.
Be Careful Not to Delete The Plums! You Can Not Get Them Back and Will Have to Start Over or Hope That You Find Enough Extra!
If I remember right, each upgrade cost one more than the next. I.e the first upgrade was one plum, then the next upgrade cost two plums, so on and so forth. So I think in total you have to collect around 21ish plums. I could be wrong on the total number of plums and feel free to correct me, but I know for sure it's 6 potion upgrades.
Edit: While playing after I got this achievement, I did find more plums in the world. So hopefully other runs should have some extra plums.

I am on my final castle/dungeon and I have picked up 4 more after completing the final vial, so there is some leeway.

Potion recipe upgrades are obtained in Fairreach, the second town of the game, by giving the chef on the left side Mega Plums. There are a total of six upgrades available in the game, and all are needed for this achievement. However, you do NOT need to collect every single Mega Plum in the game to get all recipe upgrades. There are around three extra.
Like all things on the map, there in random locations, so I can't provide locations of where to find each. Each one is on a tile all by itself though (no enemies or anything else). So, explore all overworld tiles AND dungeon tiles (you can always find one in each dungeon) to make sure you get enough to get all the potion upgrades. Once you have the sixth upgrade and unlock this achievement, you can ignore any Mega Plums you find. They just take up inventory space.
