The Banished Executioner can be found in The Executioner's Lair. To enter this dungeon, you need to beat the arena in Molten Rift.
I used the archvale lexicon +5 to defeat this boss
He will sometimes spawn beams of light and energy balls from the center of the zone. You should stay at the margin in that moment to easy evade what he throws at you. He will also try to close the gap, and you must not let him. Run away or dash away from him. Also take care of his sword attacks.
