Working: As of 14/12/2021, patch 1.1.3.0 has fixed the issue with the achievement failing to pop once the upgraded item reaches +5!
To fully upgrade an item to +5 efficiency, all you need to do is visit the Blacksmith in the second town (Fairreach). I used the snail rod as this weapon is very cheap to upgrade. I'm sure all the wooden weapons are extremely cheap as well.

I believe this is currently bugged as well as the putting 10k into the bank unless anybody else knows something different. My weapons are at level 5 at least one ranged melee and magic weapon. Is there a hidden upgrade somewhere maybe for 6?

Upgrading a weapon can only be done in the second town, Fairreach. The blacksmith is located directly up from the shops and bank, in a separate area by himself. You can upgrade any weapon in the game five times, with it getting progressively more expensive. This achievement requires you to upgrade any weapon five times.
