Easiest done in the first dungeon, as you're killing the boars. Enemies don't hide, so just use the minimap to run along all the edges. Pops when you kill the last enemy on a floor. If you don't get it immediately, don't worry, it's really easy.

I have cleared out every enemy in every floor of this game so far and this still hasn't popped. It's a 1.01 ratio, so I think it somehow glitched on me. Any way of fixing this?

Well, nevermind. As soon as I did the first level with a different character, it unlocked. >_> Strange.

Very, very easy achievement. You'll end up doing this either way while playing if you want to hit level 20, so just make sure you kill all the enemies in a level.
Run up to an enemy and press 
to attack.
