Rotting Coil is the evolved version of Toxic Stench, Poison Root is the evolved version of Crawling Root, and Elven Bow is the evolved version of Ranger Bow. This therefore requires you to get all three of those weapons in a run, get all of the necessary trinket elements in that run, and then level up each weapon to max to then evolve it with a chest, and then complete the stage.
To make this a bit easier and less random, you can play as Ker, because she starts with the Ranger Bow. I highly recommend having at least a few Rerolls and a couple Banishments purchased from the Upgrades menu before attempting this, because by this point you likely have a lot of weapons and trinkets unlocked, meaning the pool of choices will be decently large, and it might be frustrating having to hope you get what you need. It's also helpful if you've increased your weapon and trinket capacity to give yourself more room for error and make it easier to get all the right trinket types.
Thankfully, Poison Root and Rotting Coil are both quite good for surviving long enough into a tough stage to level up this much. Priority needs to first go to getting the applicable weapons, then focus on trinkets after that since you have more leeway with what trinkets you pick. With three upgraded weapons on top of others, you should be in a good position to complete the stage anyway. Finish it off to unlock this.
Alternatively, if you unlock Favorites (10G) first, you can start as Ker and favorite one of the other two weapons to ensure you get them, drastically helping with the RNG aspect of this achievement.
