Going "wounded" means going into the down state where you need to be revived. So, this isn't as bad as it sounds.
Play the mission "Last Resort" on medium because it's the shortest mission in the game having only two chapters. Just get to the end without going down and the achievement will unlock. Make sure you do not skip any of the cut scenes as well. (It is reported that you can skip cut scenes now.)
This will work in co-op or solo play. It's easy either way but will of course be a little easier with a human player.
NOTE: The achievement may void if your co-op partner goes down during the mission. Make sure both of you stay alive for the achievement.
NOTE 2: It seems in local co-op play, only the host gets the achievement. This isn't the case with online co-op assuming the achievement unlocks like it should.
GLITCH WARNING: This achievement has been reported by several people to be glitchy. There is no known reason for why the achievement glitches, but it does not unlock for some people. I will watch the comments for this solution and this thread for more information on the problem:

It can be done co-op and is easier with a human player but 100% doable solo as well. It's an easy mission and really short.

Not sure if partner wounded nullifies this. I didn't die in the Assault mission on medium, but my partner was wounded once. Didn't pop.

That could be the case. My co-op partner and I both stayed alive during the whole mission, so I wasn't able to see if that was the case.

i played with a friend and neither of us went down on your recommended mission. his popped but mine didn't. Maybe only first player and not the co-op player unlocks? We may need more people to test this theory. I don't think that should be the case though since Army of Two is pretty well known for it's co-op achievability, unlike the Lego games, which are very well known for the exact opposite... :/

I did this in co-op as well and both players got the achievement. We played on easy as well so that shouldn't hurt anything. I know the overkill kill counts in the stats can be off, so maybe the game can just be a little buggy at times?

i beat last resort 3 times without going down, but i was skipping cut scenes and going to the armory
so decided to beat the mission again but actually watching the cut scenes & not going to the armory and BOOM got the achievement it's weird

Don't skip cutscenes. I did it in co-op first, skipping the cutscenes. Afterwards I played it solo, watching all the cutscenes and it popped. So I think that it's the trick.

You CAN skip cutscenes. I played Last Resort and skipped the first cutscene and one cutscene in the middle of the mission. I then finished the mission, watched the last cutscene and got the achievement first try. So it seems that only the last cutscene is important

The lead profile is the only one that can unlock the achievement. I played with my brother and I was the lead profile and I unlocked the achievement and he did not. We backed out of the game to the title menu and he was the lead profile and I was secondary. After completing the level he unlocked the achievement as well.

If your AI co-op partner goes down and you decide to restart from checkpoint, will that void the achievement? I don't mean having him die, I mean just manually restarting from the checkpoint.

Just got it on Last Resort. DO NOT skip the cut scenes and make sure you press "A" in between the missions to show each mission stat, otherwise it will not give you the achievement! Up Vote!

Can also confirm that on my first attempt, my AI partner went down and I skipped cutscenes....didn't get the achievement.
On my next attempt, my AI partner did NOT go down, and I skipped only ONE cutscene and watched the last one of the mission.....achievement popped.
