WARNING POSSIBLE SPOILERS IF YOU HAVE NOT COMPLETED THE GAME ONE TIME YET!!!!

......OK guys so below is the way I did this with very little trouble and glitch/bug achievement free.

First beat the game on easy.
Now when you are done select NEW GAME
you will NOT lose anything!
And this should help keep the bugs away by starting new game as opposed to chapter select!

once a new game is started make sure you select INSANE

Now as a primary weapon select the M1 and take explosive ammo!!

Secondary, take the AMR 50...NO BOLT ACTION, give it explosive rounds!!! this is a MUST!

Now do not play with a human! use the AI.
(You can use a human player too but it may get frustrating)

the AI is a tank on insane and will hardly ever fall.

When you encounter brutes or during any "boss" fight....(helicopter, huge Mexican, destroyer vehicle).... use the sniper with explosive rounds! This thing will drop anything !!

Sometimes you need to activate your overkill though! for all boss fights listed above, use at least one overkill.....yours, or Ai or both and use the sniper. For the Mexican you can simply overkill, shoot him about 5 times and he's dead. For the chopper take cover in the corner near the shipping container, activate overkill and fire.

Now for part one of the final battle you need to kill elites..what I did was run around the BOTTOM square and use cover, snapping my scope to auto aim kill the elites as they appeared in view...also press down on d pad when you see the grenade, sometimes the AI can kill two or three elites with one grenade!

For part two you need to kill at least three brutes and some more elites. For the brutes.... two should come out of the bottom doors, and one on the top floor. USE THE SNIPER RIFLE on the brutes.

Make sure you save your overkill!
hopefully your AI partner will have his too and not activate it like a dumbass :-)

for the final part of the battle....overkill then run out shoot the front of the destroyer truck WITH THE SNIPER RIFLE, try to get near it when you do the damage because the turret will always be active. then get ready to run to plant c4....but try to use the AI overkill and save yours in the beginning because most times the AI will go down as you run to plant, so if he does go down, activate overkill....revive and plant the c4!

overall this should take you about 6-8 hours total. And its not really hard at all if you use the AI.

Also, I watched all of the ending cut scenes, I would suggest doing so also to avoid any achievement glitches.

THE ACHIEVEMENTS WILL POP AT THE END OF THE CUT SCENES!

either will work fine, personal preference really....I always try to choose the max damage rife as long as it has a box mag which allows at least 5 shots. You do not want to choose any sniper that has a bolt action.

When you say heavy machine gun do you mean the last of the LMG as some people refer to them as big/heavy guns or do you mean the last Assault Rifle?

Found that you could use any assault rifle, the extended clip is what you'd really like to focus on, but five star suggestion with the explosive rounds sniper rifle. That thing chews through enemies like no-one's buisness, and makes brutes a piece of cake.

With regards to the glitch at the end. I had it happen to me. After i blew the truck up, there are two cut scenes. I saw Fionas ass and the game said i failed. I did it a second time and the game froze after I killed the end boss. I had to restart my hard drive and restarted chapter 49 and.... i finally got it.

and great suggestion about the sniper rifle.

with a silence Sniper auto aims and then kills most of the time as long as you are starting in that direction. makes a lot of the distance enemies a breeze to eliminate!

After the ending glitched on me twice, I dashboarded, and then started a new game on Insane. I then quit out as soon as I could and chose the final mission on mission select. That fixed it for me.

That final mission on insane is ridiculously hard. I played through all of the game on Insane with a friend in co-op over XBox Live and we did fine up until that final damn mission. I think we tried like 30 times before just quitting in frustration. We're going to give it another try tomorrow. I've been using the M1 and AMR-50 like suggested, but my friend doesn't like using those. I got him to use the M1 but he chose another sniper rifle instead of the AMR-50. I don't know if that is whats making it so difficult or not.

If thats done, and you unlocked the horse mask and the inmate gear and didnt get the achievement, then load checkpoint 49 again on easy or any difficulty you want and kill the elites brutes and El Diabolo.

Thanks. One thing I'd add from my experience is this:
When I was playing the last mission by myself with the AI I actually went down after I planted the C4. After planting the C4 I started shooting at the turret to get Salem to aim at me instead of my AI partner. Even though I was down when the ending cutscene started I still got the achievement.

Something you may want to add to anyone having trouble with unlocking this achievement.

My game froze after I completed one of the missions, when I loaded up the game it started me on the next mission. After I completed the game, the achievement did not unlock. I looked back and it said that I had not completed the mission my game from on. I completed that mission and once again, the achievement did not unlock.

After dreading going through the campaign for the third time, I tried simply doing the last mission again, without skipping cutscenes or anything. The achievement unlocked when it should have. The funny thing was that I had unlocked the 'Insane' outfit and mask, before the achievement popped.

Anyway, what I'm trying to say is that re-doing the last mission on insane can possibly fix this buggy achievement.

Great weapon choices. For final fight, I found it easier to hide in the corner back from the column a bit. Truck can't get you and turret fire rarely hit me...only when peaking to fire.

A tip for the final battle against Salem in the truck.

Get behind a middle column (not a corner one), blindfire when the mmg is shooting at you, and fire from cover when its not. If the truck charges against the column, get back a couple of steps, so you dont get knocked down. Activate your mate's overkill as soon as you can, but always save yours. Just keep firing from behind the column.

Now, once the truck has taken a beat, plant the c4 and get to cover, and when the countdown is near zero, activate your overkill. That way you will avoid the glitch where Bravo gets killed after the credits (it happened to me like 5 times in a row).

I'm stuck on easy difficulty because I don't have the sniper yet. For some reason my partner keeps dying before I can take down the truck. He doesn't go down, he dies.

My AI keeps getting "downed" during the last cut scene even though I used Overkill and physically pushed him into cover during the C4 countdown. Has anyone else ran into this problem?

I've done it on insane and damn what a breezin'.
(Did it with a friend not the AI)
Wasn't that hard, just annoying unfair spots where the AI can shoot through things and throw grenades like an olympian gold medal winner.

But i have to add a lil something for your guide.

1:
Don't select a LMG or anything as primary, select the M1 + Explosive and this game will be way easier on insane.
This gun rapes all guys with 1 or 2 shots depending on the hit zone.
Beware of many flying body parts :D

As second, select the Sniper, the last one AMR 50 with explosive.
Enemies will die with one hit, doesn't matter where you hit 'em.
And Brutes can't handle 2 headshots or 5 body hits.

I too went down on the c4 part, after i planted it, overkill was over and the gunner hit me
but i got the achievement.
On the way towareds the end i skipped almost all boring cutscenes except for the ending.

This is not a guide just a heads up if you have gone through the game with a co-op partner and go through again for this make sure both of you select Insane in the difficulty as I just went through the game on insane but my friend was only on medium as we did not realise this happy gaming

Yeah. You both have to be playing on insane. I'd especially avoid drop in co op because even if you're playing on insane, if someone else joins it changes the difficulty to whatever they are playing on without even alerting you.

You just need to complete the missions that you did on easy. If the achievement doesn't pop then simply complete the last mission again and it will.

Really pissed off, just finished on insane, watched end credits, and nothing (played entire game solo). Replayed final mission, no luck. I go to mission select and it shows every chapter after 16 either easy or medium, which is wrong because I couldn't have unlocked insane if I didn't complete the game on medium. How do you change difficulty anyway? In the pause menu there is no option to change difficulty. I'm just about done with this game.

from a mission's menu -- not the mission select page, but the Mission page where you can Go To Mission, or the Armory. It shows

=Difficulty in a key at the bottom of the screen, how do you not see that?

You say you couldn't have unlocked Insane without completing it on Medium. That is incorrect; Insane unlocks upon your first campaign completion no matter what difficulty you played your first time through.

This glitched for me twice now. After beating the final boss and watching the end cut-scene, it popped up "Bravo was killed" and made me restart the checkpoint. I checked and it didn't count as having beat the game on Insane. Took me several more times to beat it a second time and it did the exact same thing. My partner was definitely not down / wounded either when I beat it. WTF!?!?!?!?

Mine glitched in that one random mission said Easy after doing everything on insane. Once I'd did that chapter again on Insane the cheevo popped.

Mine glitched saying Bravo had died. Second time the internet died and my rifle decided to continuously fire until the end of the whole final cut scene. Could even hear it firing when watching Salem in his prison cell. But I got the achievement, so thats fine. Cheers for the heads up on the glitches. :)

perhaps something that should be added if you do this coop online make sure you both set your difficulty to insane or itll count as the lowest difficulty on ur chapters

Additional Info:

Just got bugged yesterday while completing the game on insane. As it says here, we (offline partner and me) didn't get the achievement at the end of chapter 49. No Achievement after completing on insane. Redid on Easy, still no achievement.

WHERE DO I FIND, WHICH MISSIONS I ALREADY COMPLETED ON INSANE?

When you select a Chapter, on the right side it tells you, on which difficulty the level was finished. After we checked this, there was only 1 Mission which was saying "Normal". We selected the Chapter (42nd) and did it on Insane... No extras unlocked ... Level still saying that it was completed on "Normal"...

This issue was solved by loading Chapter 41 and we played until the end of chapter 42. In summary screen it said, that there were new items unlocked. YES finally... All we had do now was to complete again the final chapter. We did it on easy and after the Cutscenes we got the Achievement.... Hope this helps

Additional info for those who got the glitches

during the final boss fight like me. I encountered this thrice in a row ...

I read somewhere about the custom mask glitch and went to delete it (even though I'm not wearing it during the fight). Prior planting the C4, Bravo happened to be down and I activated overkill to revive him before proceeding to plant it . Manage to get the achievement this time!

After beating the game straight through on insane and not getting the achievement here is what I found to be the problem (glitch).

When you are at the main menu, select MISSIONS, SOLO, SELECT MISSION and select the first one (NEW BLOOD). Each mission has a certain number of chapters (e.g. 01, 02). To the right of each chapter you will see your STATISTICS. Make sure your BEST PAYDAY lists the difficulty as INSANE! Go through every mission and chapter making sure this is the case. I had a few spread throughout that were listed as something different other than insane. Even if you do not earn as much cash on insane difficulty, it will override your BEST PAYDAY score from the previous difficulty. Sure enough when I completed my final chapter that said something other than insane for BEST PAYDAY DIFFICULTY it unlocked the last mask and outfit. I tested what someone else said about not having to beat the game on insane difficulty after doing it once already and that is true. I beat it on easy this time and the achievement popped up.
