Option A: Michelle was shot
If Michelle is shot at the end of Chapter 1 or during Chapter 2, this is still possible. Remove the bullet to save her, and when given to option to release her or Zoe, choose Michelle. As Vince escorts her outside to the police, respond positively about the marriage during the conversation to unlock this and Lifesaver at the same time.
Option B: Michelle is not shot
During Chapter 3, Vince and Michelle will sit down and have a heart-to-heart. During this conversation respond positively by saying you want to work on the marriage.

Just wanted to add that I still got this achievement and Lifesaver even though I didn't try to get the bullet out, so that's not a necessary requirement.

Only choice that matters is “Truth is everything”. Also obtainable in other timeline during hostage release by releasing a shot Michelle and answering this way. Also queues lifesaver if done via this timeline.
