This is Akane's true ending. To get this ending, you need to have finished the game with one of the basic endings. Load the clear game state and do the post-game quests. When you return to the final battle, have Akane's trust level at 100%. You can see her trust level on her status page. After the battle is over, select "I'll stay with Akane!"

Trust levels can be raised through events that occur during the game, but to get to 100%, you'll need to use items. The special shop sells Ancient Tomes that raise trust 5% for 30 spirit stones and Special Gifts that raise trust by 20% for 70 spirit stones.
