The description is a bit vague in what this is asking for. I initially thought you would unlock clothing outfits for your player character, this is not the case.
As the game progresses your townsfolks' clothing/appearance changes, similar to how Vagrants Rest changes.
I speculate that each Townsfolks' outfit reaches its final form once you've finished their specific Journeys. I had completed the following Townsfolk specific Achievements prior to attaining the Fancy Achievement:
The Secret Achievement achievement in Ashen worth 176 pointsContinue playing to unlock this secret achievement
As soon as the cutscene for defeating the Shadow of Ashen was over the Fancy Achievement popped.
