Here are some trouble spots, areas that people tend to miss:
Valley of the Kings: Likely a glitch, but this area between Pass of the Warriors and Limnai Marsh in Lakonia (bordered on the north side by the Smuggler's Forest region of Arkadia) is the only land-based sub-region that doesn't have a label. Here's a screenshot:
Kradia Isle: Most little islands just off the coast of larger land masses are part of a sub-region on the larger mass, but in some cases even the tiniest of islands is its own sub-region. One example is Kradia Isle south of Samos:
Chytra Atoll: another small isle that is its own sub-region south of Kythera Island:

That stupid little island that you show as the second area did the trick. I spent an hour looking around the map for fogged areas before following this guide. I should have just read this earlier.

To unlock this achievement you need to clear all of the fog from the map of Greece. This is likely to be one of your last achievements as it can prove quite monotonous. If you zoom into the map you can see each region is broken into smaller areas. You simply need to enter each area a good way and that entire area will then be cleared. You do not need to travel every nook and cranny, thankfully. Be wary of some of the smaller islands, the last place I unveiled was the tiny island called 'Kradia Isle' just south of Samos. It's very easily missed.

My last unexplored location after 7 hours of searching and re-visiting almost the entire map was the “Kiteron Foothills” area in Megarid, and there were two question marks on it, apparently because of this I ignored this area. When you complete this trophy, be extremely careful when examining the map.
