EarthboundX unlocked both ending achievements at the same time, so it should be patched. I'll keep my guide just in case it won't be the case for everybody. So read this guide only if you want to be 100% sure to unlock them all in one run.

Missable achievement !!!

At the moment,if you complete the game with 100% of items and map,you won't unlock this achievement.
Only

The God of the Tower! achievement in Astalon: Tears of the Earth worth 153 pointsComplete the main game mode with 100% map coverage and 100% item collection.

So you must beat the final boss BEFORE getting the 100% to unlock this one, and defeat it again with 100% for the other ahievement.

I contacted the devs and i hope a patch will come,because i don't want to restart from scratch...

I was wondering about this, thanks for the info Clad Master, yeah I really liked the game but don't want to replay the main campaign just for this achievement.

Yeah,i did the 100% in 14 hours.It's not a fast Metroidvania,and due to the Rogue part,it's not easy to speedrun (and not recommended)

I got 100% and unlocked both achievements. So it sounds like the devs may have fixed this since 2021.

But just in case since I ran into other glitchy achievements, making periodic save backups may be a good idea. Sadly can't do that with the 5000 kill achievement though, since that tracks throughout all your playthroughs.
